Homeless Outreach Stabilization and Transition (HOST) is a multi-disciplinary team consisting of mental health, substance use disorder, and medical professionals. The team provides critical survival services and engagement to adults experiencing severe and persistent behavioral health and co-occurring disorders, in addition to homelessness.
HOST prioritizes services for individuals who are highly vulnerable, lack service connections or other support systems, and are unwilling or unable to engage in traditional service models.
Services Offered

Outreach & Engagement: Aimed at locating eligible individuals, assessing individual needs, building relationships, and providing essential survival services.

Intensive Case Management & Stabilization: Includes a full range of community support and ongoing treatment.

Resource Connections: Development and support for maintaining critical resources, including housing, psychiatric and physical healthcare, medication assistance, financial benefits, and ancillary support services.

Care Transitions: Transition support to long-term behavioral health care or other appropriate ongoing services.
